Please be advised though we are now offering online refill request this does not ensure approval of your refill, this is simply an easier way to guareentee we receive your request if you cannot reach us, it' is after hours, or weekends/holidays.

Please also note once you submit this request a nurse will contact you with updates within 24-48HRS.

 

Refill Requirements:

  • Been seen within the last 90 days | Past 90 days will require an appointment. We like to know that the medications prescribed are working for our patients so this is why we require check-up’s.

  • A current patient of our practice | If you’re a current patient with our practice you’re on the right track but if you are not our patient we cannot prescribe medications so, Join the CMG VIP (Very Imortant Patient) and become a new patient at our practice and let us show you high quality care.

  • OH NO! You’re all out | if you’ve ran out of your medication let us know and we can get that refilled ASAP!
